Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Part I, Line 7
Physician contracts provide the opportunity to receive additional payments for productivity above minimum thresholds. For the period reported, the physicians listed on this Form 990, Part VII, who received incentive compensation under such a contract were as follows: - Wyll Everett, $4,656. - Jeremy Morrison, $8,615. - Marc-Olivier Ratte, $4,632. - Timothy Shafer, $12,064. The CEO's contract provides an opportunity to earn annual incentive compensation of up to a maximum of 10% of annual base salary upon attainment of mutually agreed upon goals. This incentive compensation opportunity may be modified from time to time at the discretion of the Board of Trustees. For the period reported, Douglas DiVello received a payment of $12,614. Additionally, the Hospital's CFO also received bonus and incentive compensation of $12,900 during the 2023 calendar year, and as reported on this Form 990, Schedule J, Part II. $12,500 of this compensation was awarded due to the CFO taking on certain CEO responsibilities and job duties on an interim basis while the CEO was unavailable during this reporting period. In the addition to the incentive and productivity payments detailed above, certain individuals listed on Part VII also received wellness stipends and other non-fixed payments which have also been included in the total reported on this Schedule J, Part II, Column B(ii) as "bonuses."

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
