Mission
Home Health and Hospice Care provides trusted patient centered services that enhance each individual's independence and quality throughout life.
Activities or mission (Part I)
Home, hospice, and supportive services that enhance independence and quality of life.
-
Home Health & Hospice Care, a mission-based, non-profit agency, takes great pride in offering highly skilled, state of the art, compassionate homecare, including rehabilitation, IV therapy, wound care, advanced illness management, palliative care and hospice in patients' homes and in HHHC's 16 suite community hospice house. Following surgery, an acute illness or injury, skilled homecare is a resource that helps patients regain independence and become as self-sufficient as possible. And when medical care can no longer offer a cure, our hospice program focuses on providing patients with comfort and dignity as the end of life draws near. (Continued on Schedule O)HHHC's critical programs and services are generously supported by community donations which allows the agency to continue to care for patients who have fallen through the insurance cracks and have no resources.

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)
- Part I, Line 3
- The compensation for the Organization's President & CEO was evaluated and established by the officers of the board of directors, based on guidance from an outside compensation consultant and market survey data. Additinoally, The SolutionHealth CEO, the Chief Executive of HHHC's parent-organization and sole corporate member, shall manage the development of any proposal to adopt or modify the compensation or benefit programs of HHHC, with the assistance of the HHHC President/CEO and in consultation with the HHHC Executive Committee. Any action to approve such an adoption or modification of compensation or benefit plans must be initiated by a recommendation by the SolutionHealth CEO to the SolutionHealth Board and shall be authorized solely by a majority vote of the SolutionHealth Board.
- Part I, Line 4b
- The Elliot Health System and SolutionHealth, the parent-organization of the System, maintain 457(f) deferred compensation plans. The following individuals, each of whom qualify as either an officer, director, trustee, key employee, or highest compensated employee on this Form 990, Part VII, received contributions to their 457(f) deferred compensation plans during the 2023 calendar year as follows: - Michael Turilli: $80,692 - Brad Kreick: $195,181 These amounts have been reported within the named individuals' deferred compensation figures as shown on this Schedule J, Part II, Column C. The System historically maintained a defined benefit pension plan for certain employees, the Elliot Health System Pension Plan (the Plan). Effective July 1, 2006, the Plan was amended to close the Plan to employees hired after June 30, 2006. Eligible employees hired prior to July 1, 2006 are grandfathered under the Plan and will continue to accrue benefits as long as they remain at a participating System entity and in an eligible status. The System's funding policy is to contribute amounts to the Plan sufficient to meet minimum funding requirements set forth in the Employee Retirement Income Security Act of 1974, plus such additional amounts as might be determined to be appropriate from time to time. The Plan is intended to constitute a plan described in Section 414(k) of the Internal Revenue Code, under which benefits derived from employer contributions are based on the separate account balances of participants in addition to the defined benefits under the Plan. In calendar year 2023, no officers or other employees of Elliot Hospital received defined contributions under the now-closed 414(K) plan. Additionally, there were no defined benefit accrual adjustments for any of the Organization's listed individuals for the 2023 calendar year.
- Part I, Line 7
- At risk compensation, which is subject to review and paid annually. It is based on several predefined metrics.
Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
