Mission

HOSPICE OF NORTHWEST OHIO PROVIDES SPECIALIZED MEDICAL EMOTIONAL AND SPIRITUAL CARE TO PEOPLE OF ALL AGES - AND THEIR FAMILIES - LIVING WITH ANY END-STAGE ILLNESS IN NORTHWEST OHIO AND SOUTHEAST MICHIGAN

  • HOSPICE CARE IS CARE DESIGNED TO GIVE SUPPORTIVE CARE TO PEOPLE IN THE FINAL PHASE OF A TERMINAL ILLNESS AND FOCUS ON COMFORT AND QUALITY OF LIFE, RATHER THAN CURE. THE GOAL IS TO ENABLE PATIENTS TO BE COMFORTABLE AND FREE OF PAIN, SO THAT THEY LIVE EACH DAY AS FULLY AS POSSIBLE. THE PHILOSOPHY OF HOSPICE IS TO PROVIDE SUPPORT FOR THE PATIENTS' EMOTIONAL, SOCIAL, AND SPIRITUAL NEEDS AS WELL AS MEDICAL SYMPTOMS AS PART OF TREATING THE WHOLE PERSON. HOSPICE PROGRAMS ARE REQUIRED TO USE A MULTIDISCIPLINARY TEAM APPROACH, INCLUDING THE SERVICES OF A NURSE, DOCTOR, SOCIAL WORKER, AND CLERGY IN PROVIDING CARE. ADDITIONAL SERVICES PROVIDED INCLUDE DRUGS TO CONTROL PAIN AND MANAGE OTHER SYMPTOMS, PHYSICAL, OCCUPATIONAL, AND SPEECH THERAPY, MEDICAL SUPPLIES AND EQUIPMENT, MEDICAL SOCIAL SERVICES, DIETARY AND OTHER COUNSELING, CONTINUOUS HOME CARE AT TIMES OF CRISIS, AND BEREAVEMENT SERVICES. ALTHOUGH HOSPICE CARE DOES NOT AIM FOR CURE OF THE TERMINAL ILLNESS, IT DOES TREAT POTENTIALLY CURABLE CONDITIONS SUCH AS PNEUMONIA AND BLADDER INFECTIONS, WITH BRIEF HOSPITAL STAYS IF NECESSARY. HOSPICE PROGRAMS ALSO OFFER CARE FROM TRAINED VOLUNTEERS, WHO TAKE OVER THE PATIENTS' CARE SO THAT THE FAMILY OR OTHER PRIMARY CAREGIVERS CAN LEAVE THE HOUSE FOR A FEW HOURS. VOLUNTEER CARE IS PART OF HOSPICE PHILOSOPHY AND IS REQUIRED UNDER THE CONDITIONS OF PARTICIPATION AND LICENSURE PALLIATIVE CARE. PALLIATIVE CARE IS A HOME-BASED CONSULTATION SERVICE THAT PROVIDES RELIEF FOR PATIENTS WHO HAVE ADVANCED CHRONIC ILLNESS BUT ARE NOT ON A HOSPICE PROGRAM. AS A CONSULT SERVICE, IT IS A SHORT-TERM RELATIONSHIP WHICH FOCUSES ON ALLEVIATING THE SYMPTOMS OF A DISEASE OR THE SIDE EFFECTS OF TREATMENT.

    Expenses $19,904,835 · Revenue $21,422,765

  • HOSPICE PROGRAMS GENERALLY ARE HOME-BASED, BUT THEY SOMETIMES PROVIDE SERVICES AWAY FROM HOME IN FREESTANDING FACILITIES, NURSING HOMES, OR WITHIN HOSPITALS. HOSPICE OF NORTHWEST OHIO PROVIDES INPATIENT HOSPICE CARE AT TWO FREESTANDING HOSPICE CENTERS - 30000 E RIVER RD, PERRYSBURG, OH AND 800 S DETROIT AVE, TOLEDO, OH.

    Expenses $7,496,318 · Revenue $5,855,450

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.
