Mission and programs · 2024

Mission

Founded in 1989 to provide prenatal care for lowincome and homeless women, HPP has evolved into a dynamic family resource center with a holistic model that integrates a wide array of services to address the specific needs of each client coordinated by a team of family case managers (~50% of whom have similar life experiences as the clients we serve.) HPP offers support for families by offering a 3-pronged menu of services. Families seeking support at HPP matriculate into Housing Assistance Family Support Services and/or wellness services. These services are currently being offered both on-site and in a remote capacity.

  • Intensive Case management - HPP utilizes a holistic approach to empower pregnant women and families through trauma-informed case management services that help families work toward goals with one-on-one counseling (named internally ICM). Families who receive ongoing case management are generally involved with one of the following services at HPP: New Beginnings: Comprehensive support for women with high-risk pregnancies, including those with contributing risk factors such as homelessness, mental health issues, domestic violence and other challenges. Keeping Families Together: Individualized support for families involved with child welfare and/or substance use disorders. Bringing Families Home: Specialized housing assistance and counseling for high-need homeless families either involved with child welfare or at-risk of involvement. Domestic Violence Services: Family-centered support for those struggling with intimate partner violence. Mental Health & Parenting Services: Onsite individual, couples, family and group therapy to HPP clients, as well as parenting classes, fatherhood services and childrens playgroups. Specialized services for families and young children who have witnessed community or family violence (offered onsite through the Child Trauma Research Project).

    Expenses $7,128,407

  • Housing Assistance Program services include housing search and placement, financial assistance with move-in costs, lifting barriers to obtaining housing, budget counseling and short-term rental subsidies. HPP also offers regularly Housing Workshops to help educate families on housing processes (including search, tenants rights and responsibilities, etc.).

    Expenses $2,475,321

  • Transitional Housing Located in the Bayview District, Jelani House is a transitional living facility for up to 17 pregnant, homeless women. Residents stay at Jelani House at any point in their pregnancy, through the birth of their babies and into their postpartum bonding/healing period. While at Jelani House, residents receive case management, and are able to access groups, classes, therapy and job training. HPP also helps these families exit to permanent, safe and long-lasting housing.

    Expenses $2,178,085

  • Client Service - Daily drop-in support, crisis management, emergency services (including food, clothes, diapers, hotel vouchers) and referral services. Partners offer specialized, scheduled drop-in support such as legal assistance (Project Legal Link), preschool navigation (Wu Yee) and credit counseling (Consumer Credit Counseling Service of San Francisco). Emergency Food Pantry & Essential Item Distribution provides food (fresh and nonperishable), formula, household staples (cleaning products, toiletries). Fatherhood Support services provide group and one-on-one support specially designed for fathers. Onsite Partnerships that bring complementary services onsite/virtually to HPP families. These partnerships include benefits registration, financial support, legal counseling, preschool navigation and GED classes. Referrals and Linkages to dozens of other social service agencies for services HPP is unable to provide (e.g., substance abuse treatment programs, family shelters, legal services). Case managers coordinate these referrals for clients as part of their overall plan for self-sufficiency. These agencies refer their clients to HPP to maximize the value of the services clients receive.

    Expenses $1,859,852

  • Workforce Development - Community Health Worker (CHW) Apprenticeship Program prepares low-income individuals for meaningful careers in the nonprofit sector. The intensive 14-month program (with an education component) trains individuals in office and computer skills, client intake and record keeping, and community outreach. Trainees attend classes to earn GEDs, advanced degrees and professional certifications, participate in HPP staff trainings, and complete internships with partner agencies for additional experience and exposure to employers. This program has been an integral part of HPP since the agencys founding.

    Expenses $1,729,660

  • Wellness & Well-being Services In addition to providing one-on-one family support, HPP offers a variety of other programs and services designed to meet family needs related to health and stability. These are activities that encourage a healthful transition from pregnancy to parenting by offering a holistic and integrated array of services designed to help women deliver healthy babies, encourage mother/child bonding and acquire parenting skills that promote positive child development, including prenatal classes, group prenatal care provided by midwives from San Francisco General Hospital, alternative health services (e.g., prenatal yoga classes, acupuncture, doula support), post-partum support and family planning counseling.

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.
