Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Schedule J, Part I, Line 3
The Executive Committee of Concordia Lutheran Ministries (Parent) (EIN 20-5138278) approves the compensation of the Chief Executive Officer (CEO) annually through his employer Concordia Care Network (CCN) (EIN 47-3508524). Compensation includes base compensation, bonuses, and other fringe benefits such as health insurance. The committee uses data from state and national associations for organizations of similar size to compare compensation. Executive Committee decisions for CEO compensation are communicated to the Chief Financial Officer. Compensation for key employees is determined by the CEO. Data from state and national associations is used for comparison purposes.
Schedule J, Part I, Line 4
Affiliates, Concordia Care Network (CCN) (EIN 47-3508524) and Concordia Lutheran Health and Human Care (CLHHC) (EIN 25-0969458) participate in a non-qualified 457 deferred compensation plan for certain highly compensated employees of the Organization. The Concordia Lutheran Ministries (EIN 20-5138278) Board of Directors and CEO approve all participants who are enrolled in the plan. Contributions are deferred and will fully vest in five years. Once disbursed, contributions will appear in Box 5 of the participant's W-2. In the calendar year 2024 the following participants were enrolled in the plan: Michael Falbo, Cristen Fiffik, Keith Frndak, Brian Hortert, Natalie McKay, Hope Rouda, Karen Beilstein, and Misty Ingram.
Schedule J, Part II
All board members of Concordia Physician Practice serve on a purely volunteer basis. Any compensation to a board member by a related organization, reported on Part VII and Schedule J, is for his/her service as an employee of that organization and not for his/her service as a board member. Employment compensation for board members is as follows: Michael Falbo, CFO, Concordia Lutheran Ministries and its affiliates, Cristen Fiffik, General Council, Concordia Lutheran Ministries and its affiliates, Connor Hagey, Director of Retirement Services, Concordia Lutheran Health and Human Care, Karen Beilstein, Skilled Nursing Administrator, Concordia Lutheran Health and Human Care, Natalie McKay, Director of Human Resources, Concordia Care Network and facility based affiliates, Hope Rouda, Executive Director of Personal Care, Concordia Care Network and facility based affiliates, Misty Ingram, COO, Concordia Care Network and community based affiliates, Brian Hortert, COO, Concordia Care Network and facility based affiliates, and Keith Frndak, CEO, Concordia Lutheran Ministries and its affiliates.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
