ONE BROOKLYN HEALTH SYSTEM, INC. (THE SYSTEM), WAS FORMED IN OCTOBER 2016, AS A CHARITABLE CORPORATION ORGANIZED UNDER THE NOT-FOR-PROFIT CORPORATION LAW OF THE STATE OF NEW YORK. AMONG ITS PURPOSES, THE SYSTEM WAS FORMED TO FOSTER AND PROMOTE WELLNESS AND GENERAL HEALTH OF THE PEOPLE OF BROOKLYN, NEW YORK BY SUPPORTING AND FACILTATING THE INTEGRATION AND COODINATION OF CARE THROUGH PROMOTING AND ADVANCING RELATIONSHIPS BETWEEN HEALTHCARE INSTITUTIONS AND HEALTHCARE PROVIDERS IN BROOKLYN AND THE COMMUNITIES THEY SERVE.
