Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

PART I, LINE 4B
DURING 2015, CHNK ENTERED INTO A DEFERRED COMPENSATION AGREEMENT WITH ITS CHIEF EXECUTIVE OFFICER, WHO IS SERVING AS THE AGENCY'S CHIEF DEVELOPMENTAL OFFICER. THE ANNUAL INCENTIVE IS TO BE DEPOSITED IN AN INVESTMENT ACCOUNT UNDER CHNK'S NAME, BUT THE CEO MAY DIRECT THE INVESTMENT VEHICLE THE FUNDS ARE INVESTED. UNDER THIS AGREEMENT, THE CEO WILL BE RESPONSIBLE FOR ANY GAINS OR LOSSES AND WOULD SOLELY ACCRUE TO THE BENEFIT OR DETRIMENT OF THE CEO. IN THE EVENT ANY PORTION OF THE ACCOUNT WOULD BE FORFEITED DUE TO ANY REASON, THE UNINVESTED AMOUNTS WOULD REVERT TO CHNK AND THE INVESTMENT DECISIONS WOULD ALSO REVERT TO CHNK.
PART I, LINE 5
DURING FY 2023 CHNK ENTERED INTO AN AGREEMENT WITH OUR APRN WHERE HER PAY IS BASED ON THE AMOUNT OF REVENUE SHE GENERATES MONTHLY. SHE IS PAID 65% OF THAT MONTHLY REVENUE.
PART I, LINE 6
CHNK ENETERED INTO AN OPERATIONAL EFFECTIVE BONUS (OEB) WITH ITS CHIEF EXECUTIVE OFFICER, WHO IS SERVING AS THE ORGANIZATION'S CHIEF DEVELOPMENT OFFICER. UNDER THIS AGREEMENT, IF CHNK'S CHANGE IN NET ASSETS IS POSITIVE OR BETTER THAN BUDGETED, THE CEO WILL BE ELIGABLE FOR THE OEB EQUAL TO 20% OF THE CEO'S BASE SALARY, PAID OVER A 2-YEAR PERIOD. IF THE CEO WERE TO LEAVE CHNK WITHIN THE 2-YEAR PERIOD, ANY REMAINING INSTALLMENTS ARE FORFEITED. DURING THE YEARS ENDED JUNE 30, 2023 AND 2022, THE ORGANIZATION HAS AN APPROVED LONG-TERM RETENTION INCENTIVE THAT VESTS OVER A 10-YEAR PERIOD, AND AN OPERATIONAL EFFECTIVENESS BONUS BASED ON SPECIFIC CRITERIA, THAT IS PAID OVER TWO YEARS SUBJECT TO CONTINUED EMPLOYMENT FOR ITS CEO. A YEARLY INCREASE OF $46,550 WAS RECOGNIZED FOR THE ACCRUAL OF BENEFITS FOR THE YEAR ENDING JUNE 30,2023.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
