Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)
- Schedule J, Part I, Line 3
- THE PRESIDENT & CEO, IS PAID BY MADONNA REHABILITATION HOSPITAL, A RELATED ENTITY. THE HOSPITAL'S PROCESS FOR DETERMINING THE COMPENSATION CONSISTS OF THE FOLLOWING: THE EXECUTIVE COMPENSATION COMMITTEE ANNUALLY DETERMINES THE CEO'S BASE SALARY BY REVIEWING 990 DATA FROM NONPROFIT REHABILITATION HOSPITALS AT A NATIONAL LEVEL. THE DATA FROM NONPROFIT REHABILITATION HOSPITALS SHOULD BE FOR HOSPITALS WITHIN APPROXIMATELY PLUS OR MINUS 50% OF MADONNA'S GROSS REVENUES RECOGNIZING A NEED FOR A MINIMUM OF AT LEAST EIGHT COMPARABLE SIZED INSTITUTIONS. THE ANALYSIS INCLUDES CALCULATING THE SALARIES FOR BENCHMARK FIRMS PROVIDED BY THE DIRECTOR OF HUMAN RESOURCES.
- Schedule J, Part I, Line 5
- FOUNDATION LEADERSHIP, APPROXIMATELY FIVE, ARE ELIGIBLE PARTICIPANTS OF THE FOUNDATION'S MANAGEMENT INCENTIVE COMPENSATION PLAN. THE PLAN PROVIDES A STRUCTURE FOR MEASURING THE FOUNDATION EFFORTS IN FUND RAISING. THIS PLAN ALLOWS FOR RECOGNITION AND COMPENSATION WHEN THE FOUNDATION'S PERFORMANCE EXCEEDS BASELINE CRITERIA. UPON ACHIEVEMENT OF AN ESTABLISHED QUALITY BENCHMARK, COMPENSATION IS DETERMINED BASED UPON A FIXED FORMULA ENCOMPASSING FUND RAISING GOALS AND PARTICIPANTS' WAGES. THE FORMULA IS APPLIED EQUALLY TO ALL PARTICIPANTS.
