The Visiting Nurse Association of Indiana County dba Concordia-IRMC VNA provides compassionate home health, hospice and palliative care as an integral part of the continuum of care for persons in and around Indiana County. The program service accomplishments of Concordia-IRMC VNA are driven by the core mission to provide a comprehensive program for the residents of Indiana, Armstrong, Jefferson, Westmoreland, and Cambria counties. The agency strives to succeed with an aging-in-place philosophy by providing a wide breadth of services in the home, preventing rehospitalization, enhancing home safety and monitoring chronic disease states. The agency also works to provide the highest quality care to anyone in need, regardless of ability to pay. Tele-Health has been utilized over the past year to provide additional monitoring of patient vital sign trending, monitoring and tracking patients with comorbidities. Concordia-IRMC VNA employs a Transitional Nurse Liaison who serves the community by assisting discharge planners assess the patients' needs to obtain appropriate orders for the plan of care. This position also attends community gatherings to discuss programs and events sponsored by Concordia-IRMC VNA. The goal is to reach as many people as possible in the community to promote wellness and teach prevention. Flu Shots were provided at the community Flu Clinics held at various sites. This includes a sponsorship to the Indiana Area School District: $250 for Back to School Missions Drive and a donation to the Armstrong School District: $300 for the National Honor Society and Key Club Veterans Breakfast. Additional funding via distributions to 501(c)(3) member hospital systems at a total of $1,000,000 have expanded the like-minded mission within the geographical communities we serve. The Visiting Nurse Association of Indiana County dba VNA Family Hospice's primary focus is helping individuals to live peacefully, positively, and as fully as possible through caring, not curing. Hospice is not a place, but a concept for healthcare delivery. It focuses on creating a natural and comfortable end-of-life experience for those dealing with a life-limiting illness, all in the comfort of their own home. Whether the goal is relief from pain, desire to experience a meaningful event such as a wedding, the birth of a child, a birthday, resolving unfinished business, or is simply the hope for a restful night sleep, the team at VNA Family Hospice can provide an array of supportive services. These include, but are not limited to: nursing and related services, personal care counseling, companionship, spiritual services and bereavement care. Hospice program patients and bereavement families received services and were provided with 9,214 visits. In addition, hospice volunteers provided 630 recorded hours of service - with a dollar value of $17,152 - to program recipients, including support to the family of the hospice patient.

Also, a summer newsletter and bereavement mailings were sent offering spiritual support, coping techniques and education to hospice family members. The 15th annual Concordia-IRMC VNA Butterfly Release was held in person and as well as a modified drive through style format on August 4, 2024 at Mack Park, Indiana, PA. This memorial event was open to anyone who wanted to honor and celebrate the memory of their loved ones, whether or not they were on services with VNA Family Hospice or Concordia-IRMC VNA Home Health. 475 butterflies were sold for a donation to the Hospice for families to pick up and release at home or participate in a group release at Mack Park following a short dedication and memorial service. Concordia-IRMC VNA and VNA Family Hospice also continue to offer community grief support to children in need through the Hopeful Hearts program. Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)
- Schedule J, Part I
- All board members of Visiting Nurse Association of Indiana County serve on a purely volunteer basis. Any compensation to a board member by a related organization, reported on Part VII and Schedule J, is for his/her service as an employee of that organization and not for his/her service as a board member.
- Schedule J, Part I, Line 3
- On an annual basis, personnel committee does a study of wages using various data sources including Guidestar website data, an annual health care compensation data book purchased from the National Association of Home Care and Hospice organization, and PA Wage information from the PA Labor & Industry website. The committee presents their findings and recommendations to the board. The Executive Director was reviewed during the fiscal year associated with this return.
- Schedule J, Part I, Line 4
- Visiting Nurse Association of Indiana County participates in a non-qualified 457 deferred compensation plan for certain highly compensated employees. The Concordia Lutheran Ministries (EIN 20-5138278) Board of Directors and CEO approve all participants who are enrolled in the plan. Contributions are deferred and will fully vest in five years. Once disbursed, contributions will appear in Box 5 of the participant's W-2. In the calendar year 2024 the following participants were enrolled in the plan: Martin Trettel, Tammy Young, Misty Ingram and Elizabeth Coe.
