Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Schedule J, Part I, Line 1a
Taxable term life premiums added to taxable income were grossed up to compensate for taxes on the premiums. Grossed up amounts were taxed.
Schedule J, Part I, Line 3
The Board of Directors has adopted a written executive compensation plan and has delegated to the Compensation Committee the responsibility for implementing and overseeing compensation issues with regular reporting to the board. In setting and reviewing executive compensation for the COO, CFO, CMO and Vice Presidents, the Compensation Committee relies on market data for similarly situated hospital executives in comparable organizations. The Compensation Committee also reviews salary market information for the CEO and makes a recommendation to the Executive Committee. The Executive Committee oversees CEO compensation. The Executive Committee relies on market data for similarly situated professionals serving comparable institutions. The organization regularly commissions studies by ECG Management to ensure that the total compensation paid to Physicians is reasonable, consistent with objective market data and in the best interest of the organization. The compensation model for Physicians was adopted by the organization based upon those studies. Review and approval of all new Physician employment contracts by both the CFO and chairman of the Compensation Committee is required to ensure that all compensation packages are within the approval limits. Documentation and supporting detail are maintained for all compensation decisions.
Schedule J, Part I, Line 7
Employment contracts with officers of the organization allow for performance-based incentives. Each year, the Board of Directors articulates and approves objective, measurable performance criteria. If performance criteria are met, performance-based incentives are paid.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
