Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)
- Schedule J, Part I, Line 1a
- Health/Social Club dues of $2,000 were paid for the President/CEO.
- Schedule J, Part I, Line 1b
- As part of the CEO's contractual employment agreement, health or social club dues are included in his benefit package. Therefore, there is no written policy regarding payment or reimbursement of those expenses outside of the employment agreement itself.
- Schedule J, Part I, Line 3
- The CEO of the System received compensation from a related organization through a common paymaster. Because employees are included in the common paymaster, the CEO's salary is allocated to the System. In addition, the affiliated entities collectively establish CEO compensation by manners described on this line.
- Schedule J, Part I, Line 4
- The following individuals participated in a supplemental nonqualified retirement plan and received employer paid amount that are included in deferred compensation. 457(f) plan payments were Michael Stapleton - $88,794; Mark Prunoske - $34,881; Kurt Koczent - $31,203; and David Baum, MD - $41,665. SERP plan payments that are included in deferred compensation was Steven Goldstein - $125,000; David Linehan, MD $150,000 and Michael Apostolakos, MD - $50,000. These amounts represent employee deferrals of compensation plus investment earnings over several years that were vested and paid pursuant to the requirements of the plans. The employee deferrals were reported as deferred compensation in Schedule J, Part II, Column C in prior years and reflected, net of investment earnings, in Schedule J, Part II, Column F.
- Schedule J, Part I, Line 7
- The President/CEO, Executive VP/COO, and Senior VP of Finance/CFO are eligible for a variable percent year end incentive based on predetermined goals. The goals are operational, financial and managerial in nature. The financial goals tie, in part, to the net earnings of the organization and it's affiliates.
Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
Grants on this return
