Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

PART I, LINE 4B
PETER GAYLORD, MBA AND CHRISTOPHER RINN, OFFICERS, PARTICIPATED IN A SUPPLEMENTAL NONQUALIFIED RETIREMENT PLAN. THE AMOUNTS BELOW WERE ACCRUED DURING THE YEAR, NOT INCLUDED IN THE 2023 FORM W-2 AND ARE REPORTED IN PART II AS DEFERRED COMPENSATION. PETER GAYLORD, MBA $54,099 CHRISTOPHER RINN $50,769
PART I, LINE 7
members of the organization's SENIOR MANAGEMENT ARE ELIGIBLE FOR a management incentive performance (MIP) salary adjustment. THE MIP is RECOMMENDED BY THE ORGANIZATION'S COMPENSATION COMMITTEE AND APPROVED BY THE BOARD AFTER CONSIDERATION OF COMPENSATION ARRANGEMENTS PAID BY SIMILARLY SITUATED ORGANIZATIONS FOR SIMILAR SERVICES, COMPENSATION SURVEYS, AND OTHER MATTERS TO DETERMINE THAT NO MORE THAN REASONABLE COMPENSATION IS PAID. The overall bonus pool is calculated based on company performance in several categories (financial, quality, engagement). Within those categories, goals for different metrics (EBIDA, cash days on hand, 30 day readmissions, employee satisfaction) are established at the beginning of the year and assigned a weight. Senior leadership considers an individual's performance to determine how much to award each employee based on the overall size of the bonus pool. THE mip PAYMENTS ARE REPORTED IN PART II.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
