Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Part I, Line 7
Bonuses paid to providers are contractual agreements between the respective individuals and Pines Health Services. They are determined pursuant to the language within their respective contracts. Dr. Michaud receives a stipend with 2 payrolls per month (a total of 24 per year) for mid-level practitioners' sponsorships. He is responsible and oversees their work within his specialty. Dr. Karod receives a stipend with 2 payrolls per month (a total of 24 per year) for mid-level practitioner sponsorship. He is responsible and oversees their work within his specialty. In 2024, we restructured the department and replaced the mid-level FNP with an RN. As a result, Dr. Karod no longer receives this stipend. He did receive a total of 15 for 2024, however this will not continue moving forward. Dr. Flynn too receives a Mid-Level Practitioner Supervision stipend for each mid-level practitioner that he oversees. He is also given a monthly stipend for his Skilled Nursing Facility Duties. Dr. Flynn also serves as the Medical Director of the Tufts/MaineMed LIC Program and is awarded a monthly stipend in recognition of the expenses incurred through this program. Dr. Vagia receives a stipend with 2 payroll per month (a total of 24 per year) for mid-level practitioner sponsorship. She is also given an annual stipend for $20,000 to serve as the Medical Director for the Jefferson Cary Cancer Clinic and Hematology/Oncology services. This is broken down to pay each pay period (a total of 26 per year).

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
