9 other organizations in the IRS Business Master File list the same street address. matched by us

We match the street line and ZIP code as the IRS gives them, ignoring case and outer spaces and nothing else, so "AVE" and "AVENUE" count as different streets and this list can miss some. A shared address can mean related organizations or a shared office, but it can also be a registered agent, a mail drop or a service provider holding mail for unrelated clients. It is not, by itself, evidence that two organizations are connected.

Mission

See Schedule O.

Activities or mission (Part I)

To provide healthcare services and facilities that benefit the communities we serve.

  • Baptist Health Medical Group, Inc. ("BHMG") provides physician services and healthcare facilities, including primary care centers, rural healthcare centers, and urgent care centers to support the mission of Baptist Healthcare System, Inc. ("BHS") and its hospitals. BHMG is a comprehensive network of primary and specialty physician practices in Kentucky and Indiana that includes over 800 physicians and more than 850 advanced practice clinicians. Providers represent more than 80 specialties, including primary care and family medicine, internal medicine, osteopathic medicine, emergency medicine, general surgery and a wide range of surgical specialties offering advanced treatments and care. During fiscal year 2025, BHMG had over 4.9 million patient visits at more than 450 locations. BHMG also operates urgent care centers, rural health clinics and occupational medicine facilities. The primary care and specialty care physician services support the hospitals operated by Baptist Healthcare System, Inc. by staffing emergency departments and providing hospitalist, intensivist, laborist, and surgical care services for patients. BHMG's primary and specialty care physicians, urgent care centers, occupational medicine facilities and rural health centers offer convenient quality care services to patients and provide cost effective and compassionate patient care in communities served by BHS. BHMG physicians play key leadership roles in determining the direction and development of patient services, they spearhead innovation by leading clinical research and they help to bring advanced treatments and technology to our facilities. The Medical Group has been a driving force in helping Baptist Healthcare System rank among the top 25% nationally for patient safety, health outcomes and patient experience, while maintaining costs among the lowest third in the nation. In primary care quality outcomes, performance is in the top decile nationally, supported by strong screening and prevention efforts that help patients stay healthy. BHMG also ranks in the 85th percentile for patient experience, showing that quality care and compassion go hand in hand. Through teamwork, expertise and ongoing improvement, Baptist Health Medical Group continues to raise the standard for quality care and better health in our communities. In order to meet the health and wellness needs of communities we serve throughout Kentucky, Indiana and Illinois, Baptist Healthcare System, Inc. (BHS) provides grants to Baptist Health Medical Group, Inc. (BHMG). BHS is a nonprofit Section 501(c)(3) related entity and the sole member of BHMG. These grants allow BHMG to continue to provide support and care for Medicaid and charity care patients through primary care centers, rural health centers, urgent care centers and specialty health centers that meet a demonstrated community need. BHS makes these grants on the condition that BHMG use the grant funds only for the following activities:1. To provide care for patients participating in Medicaid and other government health programs, including but not limited to the Kentucky Children's Health Insurance (KCHIP) program.2. To provide care, for free or at discounted rates, to low-income patients who cannot afford the cost of care.3. To fund the operations of rural health clinics, primary care centers and urgent care centers that provide care for the patients described above.4. To engage in community activities and programs in response to demonstrated community needs to improve community health, including but not limited to health screenings; wellness care; vaccinations and community health education. Baptist Healthcare System, Inc. reports the grants related to the above activities on its respective IRS Form 990, Schedule H, Part I, Line 7.

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us. Earlier years are each on their own filing page.

Nonprofiteer parses Forms 990, 990-EZ and 990-PF, in a window of recent tax years. These returns are recorded from the IRS e-file index, so their figures were extracted by that index rather than read from the return here — treat them as a different source from the filings above, not as more of the same record. A dash means the index carries no figure; Form 990-T listings carry none at all. Showing the 10 most recent; the API carries the rest.

Auto-Revocation List

Not listed. The IRS publishes this list for organizations whose exemption was automatically revoked for failing to file for three consecutive years.

Publication 78

Listed as of 13 October 2026, under deductibility PC.

These are two separate IRS files on two separate schedules, shown as published and not reconciled with each other. Neither is a statement about whether a contribution is deductible today: 19,245 organizations appear in Publication 78 while the IRS's own revocation file lists them as currently revoked, more than half of those revoked before 2019. Absence from Publication 78 is evidence of nothing, and presence is not evidence either. Verify current status with the IRS before relying on it.
