Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Part I, Line 3:
EXECUTIVES ARE COMPENSATED FOR THEIR SERVICES AS EXECUTIVES OF CAMBRIDGE HEALTH ALLIANCE FOUNDATION, INC. (THE "ORGANIZATION") THROUGH A RELATED ORGANIZATION'S PAYROLL. DIRECTORS OF THE ORGANIZATION WHO WERE COMPENSATED BY A RELATED ORGANIZATION WERE COMPENSATED FOR SERVICES THAT WERE RENDERED IN OTHER ROLES. THE ORGANIZATION RELIES ON ITS SOLE CORPORATE PARENT, CAMBRIDGE PUBLIC HEALTH COMMISSION D/B/A CAMBRIDGE HEALTH ALLIANCE ("CHA"), TO DETERMINE COMPENSATION FOR THE ORGANIZATION'S PRESIDENT. THE COMMITTEE IS COMPOSED OF INDEPENDENT PERSONS. TO ESTABLISH THE COMPENSATION OF THE ORGANIZATION'S PRESIDENT, CHA USED A COMPENSATION COMMITTEE, INDEPENDENT COMPENSATION CONSULTANT, COMPENSATION SURVEY OR STUDY, AND APPROVAL BY THE COMPENSATION COMMITTEE OF THE BOARD OF CHA. CHA HAS ENTERED INTO A WRITTEN CONTRACT WITH SENIOR EXECUTIVES SERVING AS THE ORGANIZATION'S PRESIDENT. PART II: THE FOLLOWING INDIVIDUALS WERE BOARD MEMBERS AND OFFICERS WHO WERE COMPENSATED FOR THEIR POSITIONS AT THE ORGANIZATION'S SOLE CORPORATE MEMBER, CAMBRIDGE PUBLIC HEALTH COMMISSION D/B/A CAMBRIDGE HEALTH ALLIANCE, AND ITS RELATED ORGANIZATIONS. CONNIE YOUNG (Former) ASSAAD SAYAH, MD JILL BATTY ANDREW FUQUA
