Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

Schedule J, Part I, Line 1a
Fringe benefits are recorded throughout the year; however, if an employee does not have sufficient wages to cover the associated tax withholding, the Hospital performs a gross up at year end. The Hospital pays the employee portion of Social Security and Medicare taxes associated with the benefit. These employer paid amounts are treated as additional taxable compensation to the employee. Employees remain responsible for any applicable federal income taxes related to these fringe benefits, which are reported on Form W-2.
Schedule J, Part I, Line 1b
The gross up adjustment for fringe benefits is based on the Hospitals historical practice and is not based on a written policy.
Schedule J, Part I, Line 3
The Hospital contracts with an independent consultant every 3 years to perform a review of executive salaries and benefits. The most recent review was completed in January 2023. The resulting report is provided to the Board and Management for review. The Board then sets compensation levels for the CEO through a written employment agreement. The CEO is responsible for hiring the rest of the Hospital's executive team and for using comparable data studies to set competitive compensation salary packages.
Schedule J, Part I, Line 4
The Hospital provides a 457 Top Hat plan to certain key employees and executives. Some individuals listed on Schedule J participated in the plan and either deferred their own compensation or received Hospital contributions into the plan throughout the year.
Schedule J, Part I, Line 6
The Hospital has an annual incentive plan for all employees. Incentive is based on a series of operational and quality measures in order to maintain the highest standard of patient care. One of the measures is meeting the Hospital's financial budget. The incentive is not calculated on actual earnings. Other measures include Quality of Care indicators and Patient Satisfaction.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
