Mission

FOCUS ON THE DELIVERY OF COMMUNITY BENEFIT THROUGH WELL-BEING IMPROVEMENT INITIATIVES. COMMITTED TO PROVIDING FOR THE HEALTH OF THE COMMUNITY, NORTH TEXAS HEALTHY COMMUNITIES PLANS TO BE INVOLVED IN A VARIETY OF PROJECTS WHICH WILL HELP THE COMMUNITY AT LARGE.

Activities or mission (Part I)

To deliver community benefit through well-being improvement initiatives.

  • Since 2014, Texas Health has taken the lead on improving well-being in Fort Worth through implementation of Blue Zones Project, an initiative designed to improve community health and wellness through permanent changes to the built environment, policy, and social networks. In 2019, following Fort Worth's certification as a Blue Zones Community, Blue Zones Project activities moved fully under the umbrella of North Texas Healthy Communities (NTHC), a community outreach arm of Texas Health. NTHC has continued to implement best practices and expand its initiatives across North Texas through collaborations with under-resourced schools, faith communities, worksites and neighborhoods as identified by Texas Health'sCommunity Health Needs Assessment. Emphasis has been placed on creating equitable, affordable access to healthy foods and fresh produce, supporting mental health, and increasing literacy success - integrating these three priority areas with existing initiatives to positively impact social determinants of health. Key programs of NTHC include: * Good For You Pantry fresh produce * Double Up Food Bucks implementation at farmers markets and grocers * Fresh Access fresh produce distribution in community centers * School Learning Gardens * Scholastic Book Fairs and literacy support * Mental Health First Aid training * Recess, playground, and movement initiatives in schools * Culled Produce recovery and redistribution

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

The filer's explanations (Part III)

SCHEDULE J, PART I, LINE 3 - Officer Compensation
The organization relied on Texas Health Resources (THR), a related 501(c)(3) organization with centralized compensation professionals to use the following methods to establish the compensation of the organization's Top Executive Officer who is responsible for the daily operations of the entity. * The Governance committee (which includes independent persons) performs reviews and makes recommendations to the THR Board to approve the overall payment philosophy. * An Independent compensation consultant may be hired. * Compensation surveys or studies are utilized. * Board or Governance committee approves the executive compensation parameters within the executive compensation philosophy. The president is a corporate officer providing oversight of the Executive VP North Texas Healthy Communities and whose compensation is paid and determined by THR.
Schedule J, Part I, LIne 4B - Nonqualified Retirement Plan
Participation in the THR plans is made available to a select group of management and highly compensated employees who are providing services in key positions of management and/or responsibility and who are determined by the THR Board of Trustees. SERP: For the active restoration account (Account balances after 12/31/2009) SERP benefits vest while the participant is employed if the participant: * Reaches age 65 * Becomes disabled or dies, * Reaches the following years of service: 2 Years - 25%; 3 Years - 50%; 4 Years - 75% and 5 or more years - 100% Participants must be employed on Dec 1 to qualify for the current year's SERP Benefit unless separation is due to death, disability, retirement (Age 65) or early retirement (separation from service at or after age 55 with 75 years of combined age and service with the system.) SERP benefits are calculated each Dec 1. Vested balances are taxed to the employee and the net balances begin accruing earnings. Vested balances are paid in a cash lump sum within a 90-day period commencing on the earlier of death, disability, or separation from service. The deferred portion is included in Schedule J, Part II, column C. In frozen restoration accounts (account balances prior to 1/1/2010), the participant or beneficiary shall be taxed on his or her vested SERP benefits upon the earliest of: * Continued employment in THR until age 68 * Termination of employment for disability or death * Involuntary termination of employment without reasonable cause; or * Satisfying a 24 month non-compete period following his/her termination of employment. Payment follows the before mentioned events, except in the case of involuntary separation when the participant must wait 24 months to receive the previously taxed benefit. LTIP: The Long Term Incentive Plan covers 2, 3 or 4 years. Payouts are made over 2, 3, or 4 years after key performance goals are met. Benefits vest if they are employed at the end of the year the payout is made. Payments can also be made if separation is due to death, disability, retirement (Age 65) or early retirement (separation from service at or after age 55 with 75 years of combined age and service with the system.) In the case of retirement and early retirement, CEO approval must be obtained and a non-compete must be satisfied prior to the end of year payout. Retention: A retention agreement may be offered to an executive in a key position or based on succession planning needs of the system. The length of the retention period will be dependent upon the needs of the organization to retain the key executive. Payments are made within 2 months following the end of the retention period. Payouts to the following employees were made during the year for all the plans listed above. The amounts below are included in the amount reported on Sch J, Part II, Column B(iii) and Column (F).
