Mission

WE HONOR LIFE THROUGH DONATION.

Activities or mission (Part I)

GIFT OF LIFE MICHIGAN IS THE STATE'S FEDERALLY DESIGNATED ORGAN AND TISSUE RECOVERY ORGANIZATION. AS SUCH, IT ASSISTS DONOR FAMILIES DURING THEIR TIME OF GRIEF AND ACTS AS THE INTERMEDIARY BETWEEN HOSPITALS AND TRANSPLANT CENTERS TO PROVIDE LIFE-SAVING, LIFE-IMPROVING ORGANS AND TISSUES FOR TRANSPLANT. THESE "GIFTS" INCLUDE SUCH THINGS AS KIDNEYS, LIVERS, HEARTS, LUNGS, SKIN, TENDONS AND BONE. IN FACT, A SINGLE ORGAN DONOR CAN SAVE UP TO EIGHT LIVES, WHILE A TISSUE DONOR CAN IMPROVE THE LIVES OF UP TO 75 MORE. GIFT OF LIFE ALSO PROMOTES ORGAN, TISSUE AND EYE DONATION AND MAINTAINS THE MICHIGAN ORGAN DONOR REGISTRY ALONG WITH THE MICHIGAN SECRETARY OF STATE DEPARTMENT. THE CONFIDENTIAL DONOR REGISTRY DOCUMENTS A PERSON'S SELFLESS DESIRE TO SAVE OTHERS AFTER HIS OR HER OWN LIFE HAS ENDED.

  • FACILITATED 533 ORGAN DONORS, WHICH LED TO 1,119 ORGAN TRANSPLANTS, EACH ONE REPRESENTING NEW LIFE FOR THE RECIPIENT. FACILITATED 1,960 TISSUE DONORS, WHOSE GIFTS MAY HELP CANCER AND BURN VICTIMS WITH RECONSTRUCTIVE SURGERY AND MAY PROVIDE MOBILITY FOR OTHERS WITH ORTHOPEDIC INJURIES. ORGANIZED A CEREMONY THAT PAID TRIBUTE TO THE PREVIOUS YEAR'S ORGAN AND TISSUE DONORS WHICH INCLUDED A VIRTUAL OPTION FOR FAMILIES THAT COULD NOT ATTEND IN PERSON. APPROXIMATELY 800 DONOR FAMILY MEMBERS ATTENDED THE EVENT. THROUGH ITS VOLUNTEER CORPS, PROVIDED 370 SERVICE HOURS FOR PROMOTING DONATION. MORE THAN 206,000 NEW NAMES ADDED TO THE DONOR REGISTRY, BRINGING THE STATEWIDE TOTAL TO NEARLY 4.7 MILLION RESIDENTS REPRESENTING 57% OF THE ADULT POPULATION IN THE STATE. CONDUCTED 750 CRITICAL-CARE IN-SERVICES THAT EDUCATED MORE THAN 15,000 HEALTH CARE INDIVIDUALS ABOUT THE BEST PRACTICES OF ORGAN AND TISSUE DONATION. THROUGH ITS IN-HOUSE LABORATORY, PERFORMED 63,000 TESTS ON OVER 900 POTENTIAL DONORS AND OVER 1,000 OUT-OF-STATE DONORS. THE TESTS ENSURE THAT PEOPLE RECEIVING THE TRANSPLANTS HAVE THE GREATEST CHANCE OF SUCCESS.

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.

Schedule J, Part I, Line 6a Compensation contingent on net earnings of the organization
THE INCENTIVE COMPENSATION PROGRAM IS DESIGNED TO MOTIVATE AND REWARD ELIGIBLE EMPLOYEES FOR THE ORGANIZATION'S PERFORMANCE AND THEIR CONTRIBUTION TO THE SUCCESSFUL ACHIEVEMENT OF THE ORGANIZATIONAL FINANCIAL AND OPERATIONAL METRICS. CERTAIN MINIMUM FINANCIAL THRESHOLDS MUST BE MET FOR ANY PAYMENTS TO BE MADE. A MAXIMUM LIMIT OF THE PAYMENTS IS ALSO ESTABLISHED. EACH YEAR, THE CORPORATE PERFORMANCE GOALS ARE SET AS PART OF THE BUDGET PROCESS AND APPROVED BY THE BOARD FOR THE UPCOMING YEAR. LONGEVITY BONUS: THE LONGEVITY BONUS PROGRAM IS DESIGNED TO MOTIVATE AND REWARD GIFT OF LIFE EMPLOYEES FOR THEIR CONTINUED SERVICE TO THE ORGANIZATION. THE LONGEVITY BONUS IS PAID OUT ANNUALLY IN JANUARY AND IS EQUAL TO A PERCENTAGE OF THE EMPLOYEE'S ELIGIBLE BASE WAGES, DETERMINED BY THEIR YEARS OF SERVICE. EXECUTIVE BONUS: LIKE THE INCENTIVE COMPENSATION ROGRAM, THE BOARD SETS GOALS FOR THE EXECUTIVE LEADERSHIP TEAM BASED ON THE ORGANIZATION'S PERFORMANCE. A MAXIMUM LIMIT OF PAYMENTS IS ALSO ESTABLISHED.

Part I's answers as filed. A line not listed was left unanswered; on lines 1a and 3 that means the box was not checked.
