Mission
Fairview Physician Associates (FPA) is an integrated practice network of more than 4,000 providers. As a physician-and clinic administrator - led nonprofit, we work to provide personalized care that improves health outcomes of individuals in our defined community, while providing quality care, exceptional patient satisfaction and reducing total cost of care.
Activities or mission (Part I)
The primary exempt purpose of Fairview Physician Associates Network is to advance the health of the community by continuously improving the quality of health care services delivered to individuals.
-
Fairview Physician Associates Network (FPA) currently is accountable for providing a broad array of healthcare services for over 525,000 individuals through our contracts with purchasers and payers. The network assumes risk for over 438,000 member lives in shared savings incentives, including 120,000 member lives in defined network products. Although many of FPA's health improvement and population management initiatives have been developed to increase quality, improve the patient experience, and decrease cost to the individuals served through our shared savings care system contracts, many of the benefits accrue to the entire population served through FPA affiliated clinics and Fairview hospitals (clinical integration).

Compensation is as filed in Part VII: reportable pay from this organization and from related organizations, other compensation, and hours per week. The total, marked calculated, is our sum of the three dollar columns, with a blank column counted as $0; the form itself has no total. On a Form 990-EZ the columns are Part IV's instead, and on a Form 990-PF Part VIII's — compensation, contributions to benefit plans, and expense account — and the total sums those. A Schedule J breakdown is as filed: base pay, bonus, other, deferred compensation and nontaxable benefits from this organization, with any from related organizations beside each, and the filer's own total. It is shown with the Part VII listing of the same name on the same return. The form gives no key between the two, so that pairing is ours, and is marked matched by us.
