TRINITY HEALTH CORPORATION USED THE FOLLOWING METHODS TO ESTABLISH THE COMPENSATION OF MERCYONE GENESIS IL'S PRESIDENT: - COMPENSATION COMMITTEE - INDEPENDENT COMPENSATION CONSULTANT - FORM 990 OF OTHER ORGANIZATIONS - WRITTEN EMPLOYMENT CONTRACT - COMPENSATION SURVEY OR STUDY, AND - APPROVAL BY THE BOARD OR COMPENSATION COMMITTEE

THE FOLLOWING INDIVIDUAL RECEIVED SEVERANCE PAYMENTS IN CALENDAR 2024. THIS AMOUNT IS INCLUDED IN COLUMN B(III) OF SCHEDULE J, PART II: JASON ENZLER - $57,747 THE FOLLOWING ARE PARTICIPANTS IN A TRINITY HEALTH SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN (SERP) IN 2024. THE PLAN PROVIDES RETIREMENT BENEFITS TO CERTAIN TRINITY HEALTH EXECUTIVES SUBJECT TO MEETING SPECIFIED VESTING AND EMPLOYMENT DATE REQUIREMENTS. PARTICIPANTS' VESTED BENEFITS WERE PAID OUT IN 2024, AND THEIR NON-VESTED BENEFITS FOR 2024 WERE ACCRUED. THE FOLLOWING PAYOUT FOR 2024 FOR THE PLAN IS INCLUDED IN COLUMN B(III) OF SCHEDULE J, PART II: ROBERT RITZ - $324,571 THE FOLLOWING ACCRUALS FOR 2024 ARE INCLUDED IN COLUMN C OF SCHEDULE J, PART II: KURT ANDERSEN - $185,526 CHRISTOPHER CROME - $5,000 THERESA MAIN - $3,928 JORDAN VOIGT - $71,756 THE FOLLOWING INDIVIDUALS PARTICIPATED IN A SUPPLEMENTAL ROTH INDEXED UNIVERSAL LIFE (IUL) PLAN IN 2024 SPONSORED BY MERCYONE GENESIS IA. MERCYONE GENESIS IA MAKES AN ANNUAL CONTRIBUTION TO THE IUL ON BEHALF OF THE PARTICIPANTS.
